I got mine today that I ordered from the Cosmo website.  They came in a box, much bigger than expected but with padding.  All are in terrific shape.  The subscribers issue (mask, backwards in chair) does have the white address block. So I can only assume all of those particular covers will be the same having the white block. Just wanted to post in case anybody is wondering and yet to receive their box.

I see this mag everywhere in stores right now. My favorite is the one with the black bunny ears. I don't buy it because (1) I already have TONS of M mags and have quit collecting years ago and (2) ignore point 1 because I'm saving up for the subscriber's edition of the recent MOJO magazine.
