A lot of those systems have double cassette players. Another possibility that someone tape over part of the album to include that song if the person bought the tape used. The thing is; "Up Down Suite" wasn't recorded until the Erotica era. It stems from the song "Goodbye of Innocence". If I recall, this song inspired Madonna and her producers at the time to do the cover of "Fever".
  
  It is indeed possible that it's a fake or someone recorded over the actual tape, but also the cassette for Like A Prayer was reprinted in cassette in 1993 in Europe with other early tapes so it could be a misprint, who knows!
