A woman's body should never be tagged as gross. Just saying. We all come from them. We must worship women and their bodies.

Is she naked in the 1994 Esquire shoot ? I remember the bra and thong, but not naked pics... her first actual topless photo ws by Helmut Newton for Vanity Fair in 1990, though.
 
Anyways... I think the very reason she's doing it a lot lately is because it still generates reactions like yours.
Now, it might sound like I'm one of those Madonna-ass-kissers, but I think she actually makes a point here (or any other 56-57 celebrity acting the same would, for what it's worth). 
She first felt like acting provocatively in spite of her age, and she got a shitstorm for it. Is she known for backing down ? Nope. She actually does it even more, maybe a bit too much. But she's entititled to it, and that's a great in-your-face way to say "fuck you" to ageism.

the 00s were something else (the whole "I'm a mom, wife and english duchess" thing) but the was provocative in the 90s : showing her breasts at a Gaultier fashion show, her outfits at the ToD premieres were quite revealing, let's not even mention the whole S&M SEX premiere party thing... 
 
She watered it down a bit when she got all this backlash though.
 
But now, the fact that it bothers people is a good reason for her to do it, I think. As there is nothing wrong with a 56 yo woman being sexy and provocative.

Actor Terrence Howard, who starred in Dead Presidents and Mr. Holland’s Opus and is currently the star of Empire, will be seen up front and centre in Madonna’s new music video Ghosttown, Noise11 reports.
 
Howard once had had his own album Shine Though It in 2006. It reached no. 31 on America’s Billboard chart. Howard plays record company mogul Lucious Lyon in the hit drama Empire. His character is a former drug dealer turned hip hop mogul and the CEO of Empire Entertainment.
 
Madonna is making her new video with Howard this week in Los Angeles, while she is in town for a week of guest spots on The Ellen Degeneres show.
 
The video is being directed Jonas Äkerlund, who also directed Ray Of Light, Music, American Life and Jump.

My mum sat me down and showed me pictures of all the drugs out there, what they did, the ups and downs.  She then went on to explain what could go wrong, and that she knew that she'd never be able to stop me doing what I wanted.  Her only rules were "not in my house" and "if the police ever come to my door for you, there's no protection, you brought it upon yourself".  I give Madonna real kudos for having what sounds like a similar conversation with her kids.  At the age when you're likely to try these things you really aren't going to listen to a blanket (drugs are bad) message.
