Does Taylor have 400 million records sold?  Has she had a successful career for 40 years?  Any awards for best actress?  Does every female pop artist copy her style and concert script? Does Taylor Swift reinvent herself for every era?  Nope!

All I know is something off and I agree with @Prayerhow it's odd how this photographer is just getting around releasing it now.  Also, in the age of AI and photoshop when many fans question the tinsy thing that is off, I'd think it's pretty understanding why some would now too. 
Also, people lie.  They will manipulate and even bring things forth, to try to perpetuate that lie even further.  If someone is trying to sell a book, it's much easier to do so with an unseeen, unpublished image or dish on something not known than not.
No doubt, whoever it is, there is a definite likeness to Madonna. I see it.  But I just can't 100% be convinced it's her.

Nah, every fibre of my being is saying it's Paula. I know M. That's not her. 
The gap in the teeth is a little bigger in the right pic. The amount of tooth shown is different. The top lip is thinner too. If her face were slightly more to the right (like the left pic) it would look even thinner but you can see the side more in the left. 
Bottom lip in right is full in the middle and 'pinches' in. It looks full throughout in the left pic and doesn't pinch in. 
My verdict: It's not M.

You gotta admit while the distortion looks natural on the Mercury pic it's looks very unnatural on Madonna, look at the right pointy side of the logo ! And that sharpness on the logo zone compare to the rest of blurry picture... Still strange.

I mean, if it's a fake and he's sold it as an authentic one, that's what he would say. The photographer.
Let's be honest: a Madonna picture automatically boosts the project and brings more attention to it.
On the other hand, why would he keep an unreleased Madonna picture from Danceteria until now? He would have made profit from it much earlier.

"Yep, that's me, I had been dancing for an hour so my arms look ripped. The mask flattened my nose a bit, as masks do. And oh my god, the picture is so grainy it makes me look old."

It must mean something when more than half the fanbase feels there is something off with a Madonna pic right ?

I think what disturbs me on the photo is what you pointed out on the tshirt. It looks pulled or altered which raises some questions?
