I own one, I bought it to John Heiden, he worked in the making of Lucky Star test pressing artwork. Later, John and Jeri Heiden worked with Madonna several times

I will ask my friend for a photo. She framed it. She also has a letter from the person who got it explaning all the story. That person worked in that proof artwork and with Madonna in later projects

They pressed 25 proof copies of the album artwork. It was presented to Madonna and she rejected it. All copies were destroyed except 3. A friend of mine owns one
