Mercy is definitely playing live. Those supposedly "unplayed" notes are actually percussion sounds from the backing track she's playing to. The five or six piano notes heard in the studio recording before the first verse begins are omitted in every single performance on this tour. 
However, she's not playing an actual acoustic instrument. It's an average-sounding keyboard built into a mock-up of a grand piano body. If you look closely, there's no functioning pedals but simply a standard keyboard pedal hooked to the built-in keyboard she's playing. There's also no hammer and damper movement whatsoever during her performance. 
To properly mic an actual piano for one single performance would be a nightmare scenario for any sound engineer. In order for an acoustic piano to pick up as little surrounding noise as possible (sound disturbance being immense given the position of the B-stage and the nearly 360 degrees placement of the sound system), the lid would need to be closed at all times. There are specific piano microphones that perform miraculously under such circumstances and are able to bypass a lot of distracting resonance from drums on stage and audience noise, but obviously the lid is wide open for most of the performance. 
Settling for a MIDI-controlled mock-up makes sense since wheeling around an actual grand piano from underneath the stage wouldn't be economical due to its weight, transport needs and tuning delicacy. In an unstable A/C (or lack thereof ) environment, a grand piano would detune within minutes and sound off very quickly after being tuned. 
That said, Mercy certainly is a very good and notably skilled player. She's very consistent in her interpretation of Mayer's "Le regret" but she does add a little extra nice grace note here and there from time to time. She also plays with a sense of urgency and fervour that would be nice to hear on a future studio album of the Madonna family band...

I hope we get confirmation soon. I've already booked a room, bus tickets and got a leave from work. If in the end was just a rumour, at least I will be able to stroll around the boardwalk drinking coconut water like an sad Helena from Rio. (That's a recurring telenovela character reference)

I would be thrilled to see Madonna put on such a massive event with hundreds of thousands of people. However, I honestly find it very unlikely. First of all, an outdoor concert at that time of year is unthinkable. It's a time when concerts are held in arenas because the temperatures are not high and there's a risk of rain. If she were to do it, it would be around December, but that's too late.
Rod Stewart set a record in '94 by holding a New Year's Eve concert in Rio De Janeiro. It was a free concert that drew an audience of 3.5 million people. However, it was for a different occasion, hence with a much larger potential audience and organization. And most importantly, it was during another season (the Brazilian summer).
Moreover, this show can't be replicated exactly the same outdoors. Yes, it can be adapted for an outdoor stage, but there would definitely need to be some changes. So, I don't know, it seems like a made-up story to me. Then again, I hope I'm wrong.

Wouldn’t it be great if she simplifies the show, scales it down, takes out a bunch of songs and then films it in the rain with her hair blowing everywhere for the the dvd. The meltdowns will be sensational .

She can prepare a simplified version of the show. Pick the strongest songs and the settings. Use some of the backdrops in bigger screens.
Remove the moving platforms etc and strip it down to some stage, catwalk and sides.

It is possible - she was secretly rehearsing her special Australian show Tears Of A Clown during the Asian leg of RHT so it's possible she is preparing or plans to prepare a one off show for her loyal Brazilian fans.
I do agree it's risky and security will need to be high to play Rio
